Elwood (Dan Aykroyd), uno dei due fratelli Blues, esce di prigione e viene a sapere della morte del fratello Jake (John Belushi) e di Curtis (Cab Calloway), figura paterna ai tempi dell'orfanotrofio. Elwood viene contattato ancora una volta dalla Suora Mary "Pinguina" Stigmata, che questa volta gli chiede aiuto per raccogliere fondi e costruire un ospedale per bambini. Elwood riunisce così la band ancora una volta, con il figlio illegittimo di Curtis, Cab (Joe Morton), e un giovane orfano di nome Buster (J. Evan Bonifant). La Blues Brothers Band al completo partecipa poi alla "Battle of Bands" di New Orleans per vincere il premio finale e finanziare la costruzione dell'ospedale.
